Patient Portal
Visit our patient portal to view upcoming appointment, check a balance, access reports, complete forms and upload documents.
When Regsiterring
The date of birth required for registration is the client's date of birth, not the birthdate of the responsible party.
Viewing Appointments
Click on "Appointments" to view upcoming appointments.
Accessing Forms
Click on the "Forms" button to complete forms sent to you by Project CHILLD.
Adding Insurance Card
Click on "Upload Documents" in the purple bar/menu, and add pictures of the front and back of your insurance card. These will be uploaded directly into your chart, facilitating billing your insurance company.
Accessing Reports
Click on "Records," then "Docs and Images" to view reports.
Cheking Balances
Click on "Bills" to check an account balance. We do not offer online payment at this time. Please contact our office to place a card on file for payments.
